I think the Dwyer part number you want is the DH 288, Medium Halyard Organizer Plate.  The DH 288 is made from electropolished stainless steel plate, mounts under the mast tabernacle using the same fasteners as the tabernacle, & has eight 1/2" ID holes for attaching halyards & lead blocks.  The eight holes are arranged four per side.  The mounting surface of the DH 288 is 3-1/2" wide X 5-3/4" long.  I don't have a current price.
