Jay,

OK, I looked thru my entire 1991 Dwyer catalog & I came up up several
possibilities:

DH 601 Mast Eye: Looks like a HD stainless steel plate bent in 3 places to
                              approximately fit the mast curvature with a HD
stainless
                              steel ring welded to the plate.  I would use
this part for
                              a fixed spinnaker pole mount on the mast.
Uses 4X#10
                              machine screw fasteners - 2 per side

DH 602 Mast Eye: Looks like DH 601 but sized to fit a smaller mast.  The
                              DH 601 would be the one that fits an R-22
mast.

DH 312 Bail: Looks like one piece of stainless steel strap with 3 bends in
it.
                     The outer two bends are meant to fit the mast curvature
&
                     could be slightly adjusted to fit many different sized
masts.
                     The center bend section forms a U-shape like an
eyestrap.
                     Uses 4X#10 machine screw fasteners - 2 per side.

DH 856 Jib Hound: Looks like DH 312 except that, instead of the center
                               section being an eye strap, the center
section is the
                               female portion of a clevis.  Hole is sized
for 3/16" OD
                               clevis pin.  Uses 2 X 1/4" & 2 X #10 machine
screw
                               fasteners - 1 of each per side.  The 1/4"
fastener holes
                               are towards the center of the mast & appear
to be
                               intended to accept a single thru bolt which
goes right
                               thru the mast.

Jay, these were the only pieces/parts which were anywhere close to your
description.
